​Forms and Documents

Act 1982-256 mandated a forms management program to ensure commonwealth agencies use only the forms necessary for efficient operations. While many paper forms have been eliminated or replaced by electronic or Web-based forms, the goal of efficient and effective data processing and operations remains.
 
There are three distinct groups of forms:
  • Forms requiring input from the public
  • Forms requiring input from within a commonwealth agency
  • Forms that are commonly used by several or all commonwealth agencies
 Forms in the first two groups are included in agency forms management programs. The third group, known as standard, common-use forms, is controlled by the Department of General Services.

Standard (STD) commonwealth forms are published by the Department of General Services, Bureau of Publications and can be searched online by number and description at Commonwealth STD Forms (pa.gov)​.

 Other frequently used forms are available for download by clicking the links below. If you can’t locate a form on the DGS Bureau of Publications website or in the links below, look through the options on our Services page or visit the Helpful Websites section for additional information.

Forms for Vendors

PA Electronic Payment Program Enrollment (PDF) – Enroll to receive payments from the commonwealth electronically. If you have already enrolled and need to make a change to the information we have on file, please visit the Direct Deposit and E-Remittance page for instructions.

Electronic Addenda Enrollment (PDF) – Eliminate paper remittance advices and receive information about commonwealth payments when the funds are deposited at your designated financial institution.

 

Forms for Agencies

User Security Authorization Request (PDF) – Request access to commonwealth resources such as Docfinity or the Contractor Responsibility Program System.

CRP Designation Form (PDF) – Update the names of executives at your agency responsible for approving the debarment of contractors or managing your agency’s contractor responsibility program.
